"Le" refiere a un animal pero, ¿"la" refiere a la hermana? ¿Tiene el significado de "she"?

Sí, Kala, "la" en este caso hace referencia a la hermana.
Sería como "who" en inglés:
☛ It was my nine-year-old little sister 'who' gave him that name.
También se podría sustituir "la que" por "quien" así:
☛ Fue.mi hermanita de nueve años 'quien' le puso ese nombre.
