what does "sam hell" mean? what the sam hell? I saw this in a cartoon. IT seems that it is equal to WHAT THE F**K. SO where does it come from??? ;)

It's 'Sam Hill' and Wikipedia gives a good explanation. It's much more polite than the other expression you mentioned.
