* -고 나서 = -를 한 후에 = since/after doing something (a state/action continues until now).
* -고 나서부터 = -를 한 다음부터 계속해서 = '-고 나서'를 더 강조하는 말 = ever since doing something.
* 나다 here means "coming out of, in the aftermath of (an action or event)".
* V + -ㄴ 이래로 = -ㄴ 이후 지금까지 = since V-ing (there has or hasn't been an occurrence of something).
*이래 is a formal term for "during the time since ...".
예)
=> 영철 has refused to meet anyone since he failed the examination.
=> An incident like this has never occurred since the school's founding a century ago.

Well, as a native Korean, I do not find the different meaning from those two expressions as they are used for 'after', though, I think the first one ~고 나서부터 can be used to describe some specific reason or time why you have done, chosen or changed.
Meanwhile, the 이래 of ~한 이래로 indicates "from the past until now" you mention to explain the current situation.
You can use both expressions to describe your activities and behavior after you made decisions.
