I need to fill a form that is in German, but have an English translation. I am confused about a name and a surname. Could you please tell me what is what?)

"Vorname/First name" is, well, the first name, i.e. the given one.
"Name" in this context is the family name/surname, other similar forms might have the explicit terms (Familienname/Nachname) there.
