I would panic as I tried to talk, I could not find the words. My heart would start to raise and my language speaking ability would just drop. I'd start to make mistakes. I was not able to communicate well althought I can write 90% correct English.

I propose that you consider working with songs as study material when you
practice on Skype with a practice partner etc.
It eliminates the burden of spontaneous speech.
Reading with your partner from a text, is a much better way to practice and develop confidence with the language.
I find that published essays from the Internet serve for good reading material.
After reading a text together, any discussion which follows the reading, will use much of the same vocabulary that was read in the text, which makes dialogue much easier for the student.

The problem happens because we do not think in phrases. Try to get more ready-made phrases in different situations from a conversation book with its audio materials and thematic vocabulary words. Speaking is different from writing. Although both are active skills, speaking needs spontanity and quick relevant words to support our topic. Try to get immersed in the language and practise associating words in our phrases.
My level is B2 which means I am like you but I can speak. I do mistakes usually with grammars but there are no problems with speaking.

The same things are happened to me (but without heart). At first, I wanted to think that it's related to my personality (introvert etc) and I can't talk because I can't find appropriate words even in my native language. But then I realised that I just don't have enough practice and skills :)
