How to order a question ..
You say “tu dove vivi?” can you say “dove vivi tu?” or “dove tu vivi?”
And is the order the same for the formal form? So we say “lei dove vivi?”

in italian you can order a question as you like the important is that you add "?" the esclamation mark at the end .

The examples that you provided are good .The order for the formal forms are the same but the verb is wrong ....... :

Lei dove vivE*    ?    or.... Dove vive lei    ?    Dove lei vive ?

Other forms aren't common. ( The subject is not required in Italian )
