How would you interpret this sentence? How would you interpret this sentence ‘I saw the doors of the elevator closed, and the grill shut and the fourth-floor button pushed by the porter’? I think it is clear that the porter pushed the button for the fourth floor. What is confusing to me is whether the porter closed the doors of the elevator and shut the grill. According to the sentence ‘The man holding the feet opened the door’ in the last passage, it seems, unlike today’s elevators, the doors of that elevator couldn’t open automatically. So I guess one had to close the doors of that elevator too. So back to the sentence ‘I saw the doors of the elevator closed, and the grill shut and the fourth-floor button pushed by the porter’, I guess the porter closed the doors of the elevator, shut the grill and pushed the button for the fourth floor. Yes, "closed by the porter". Otherwise, the sentence would go, "I saw the doors of the elevator close." It's quite eccentric English. "The porter closed the doors, shut the grill and pushed the button for the fourth floor" would have been more natural. And also, "the man holding the feet opened the door." So the door is opened by hand. Why do we have plural "doors" and then single "door". God knows!

It was probably the porter. This is an old style elevator, so the grill and doors had to be shut by someone since it didn't happen automatically. Since it is noted that the porter pushed the button, he probably also shut the doors and the grill. But this sentence's use of passive voice makes it unclear who did it.
