Another question just hit me: 1. Urinary tract infection is suspected of causing his fever. 2. His fever is caused by A suspected urinary tract infection. 3. His fever is suspected of being caused by A urinary tract infection. Do I need "A urinary tract infection" in all of these sentences? How do they all sound? Thank you very much. :)

Yes you do need the words every time. All your sentences are grammatically correct, but you need the capital A only when it’s used at the beginning of the sentence. But option 1 would be the one I would use for it’s brevity and clear conveyance of information

Yes, you need A. If you were talking about several infections, or infections in general, you would use INFECTIONS with no A, but here you are talking about one specific infection.
